No small engineering feat. It appears he capped the cylinder base on a donor engine to use the transmission, with a belt drive through the crank replacement. And then adapted a right angle gearbox in lieu of a sprocket for the final drive.

The right angle gearbox appears to me to be the differential from a quad. Of course if it's truly a differential then the builder would have had to weld it or otherwise lock it.
